Thank your interest in Centerpoint Wellness Northwest! When booking for the first time, you will be required to enter your credit card information in order to book an appointment. You will not be charge unless there is a late cancellation or no show. *We require 24 hours notice to change or cancel your appointment, failure to do so will result in a $50 no show/late cancellation fee.

For Patients using insurance, please provide us with a copy or photo of your insurance card and current prescription or referral via email or fax before your first appointment. We MUST have a referral prior to your time of service to bill insurance, regardless of plan.
